The catastrophic chain reaction caused by one loose wire
Investigators have revealed that the collapse of Baltimore’s Francis Scott Key Bridge was caused by a single loose wire on board the container ship Dali. A small silicone identification sleeve had been heat-shrunk over a ferrule, increasing its diameter and preventing the conductor from fully seating in its terminal block. This weak connection caused a circuit breaker to open unexpectedly, shutting down propulsion and steering as the vessel approached the bridge.
